Dubois County Health Department to Hold Late-Night Pfizer COVID-19 Vaccine Clinic for Children Aged 5-11
Local Sources - The Dubois County Health Department will be holding a special late-night clinic to
accommodate the 5–11-year-old pediatric patients wanting the Pfizer Covid-19 vaccine.
The clinic will be held at the Dubois County Health Department from 3:30-6:00 pm on
Tuesday November 9, 2021.
Anyone 5 and older is eligible to receive Pfizer Covid-19 vaccine. A parent needs to be
present for anyone 5–18-years old. If this is not an option, you MUST complete the
CONSENT FOR COVID-19 VACCINATION FORM attached below. A copy of a parent’s ID
and insurance card must be included.
